A Message from the Registrar's Office:

Proof of residency must be
shown using the table from the residency verification table at the bottom of
this web page.
Your student will NOT be registered without all 3 documents.

Up-to-date immunizations,
including varicella if your student is coming from out of state. If any
immunizations are missing your student will NOT be allowed to start school*

Proof of birth. A copy will
be kept in the student’s file.

For all students in grades 9 – 12, an unofficial transcript is required. For incoming 9th
graders, you must submit a report card.

If your student transfers in
after school has started, a withdrawal form from the former school will be
required. The withdrawal form MUST have grades-to-date.

information has been collected, PLEASE call Kris Kobata, the Registrar at (925) 682-7474
ext. 3109 to make an appointment.
Registration is done by APPOINTMENT ONLY!

RESIDENCE
VERIFICATION
Clayton
Valley
High School
is reminding all parents,
guardians, and students that all families are required to provide three forms of
documentation to verify residency within the school. To verify proof of
residency, you will need to provide three forms of documentation from the list
below, one of which must be a photo ID.
Original documents must be presented but the school will not be retaining any
copies.
Must
provide one (1) from each column.

Rental/Lease Agreement with parent’s name,
student’s name and address, as well as manager or owner’s name and
phone number

Please note that all information provided by
parents/guardians must be true and accurate.

incorrect or not true will terminate the student’s enrollment.
