REQUIRED FORMS AND READING MATERIALS FOR ATHLETES AND PARENTS 2012 Athlete Check List –List of important items athletes must address prior to the start of the season. Follow this list to be eligible for the 2012 season! 2012 MDUSD Athletic Clearance Forms – Athletes MUST have this form completed with medical insurance and emergency contact information, as well as a doctors signature and turned in to the coach in order to be eligible for participation. Forms can be obtained in the main office, PE Department, or in Student Service South. You can also access the form through the CPHS Boosters website. If you participated in a Fall or Winter Sport, please obtain a copy of your existing Athletic Clearance Form from your coach or the Athletic Director. 2012 Track & Field Athlete Information Sheet–This 2-page form should be printed, completed and turned in to the coach on the first day of practice. Be sure to note any potential schedule conflicts on this form. 2012 Athlete Apparel Order Form -Athletes must have the current year team shirt as well as the ASICS team Sweat Pants in order to participate in team competition. One shirt and a pair of team socks is provided with your team contribution. If you need to order the required ASICS team sweat pants, or the ever popular matching Team Jacket with the embroidered logo, additional shirts, please print this order form and return it with payment by the February 16th deadline. 2012 Parent e-Script Sign-Up Form–All Falcon Track & Field families should have their Safeway or other club cards registered with e-Scrip! Annual renewal is required for most cards. 2012 Track & Field Athlete Goal Sheet– Individual and Team Goals for each cyle of the season should be articulated on this form and placed in the athletes team folder. 2012 Parent Meeting Agenda–The 2012 season parent meeting is scheduled for Thursday, February 16th, at 5:30pm in the CPHS Multi-Use Room.
