Leave the following information:
1. Your name and relationship to the student
2. Student name and grade
3. Date or dates of absence
4. Reason for absence
5. If the student is to leave early, indicate the EXACT TIME of early leave and if the student is returning to school the same day.
If you are unable to call, send a note to the office. Make sure you date and sign the note. Absences not cleared within 72 hours after the student returns are recorded as UNEXCUSED in the permanent record.
